I, the undersigned, hereby voluntarily and willingly participate in basketball-related activities organized by Orlando Boom Inc. I acknowledge that these activities may involve physical exertion and potential risks, including but not limited to injury, property damage, or other unforeseen circumstances.
In consideration of being allowed to participate in these activities, I agree to the following terms and conditions:
1. Assumption of Risk: I understand and acknowledge that participating in professional basketball activities involves inherent risks and hazards. I am aware of these risks and voluntarily assume all responsibility for any injury or damage that may occur during my participation.
2. Release of Liability: I hereby release, waive, and discharge Orlando Boom Inc. its officers, employees, agents, representatives, and volunteers, from any and all liability, claims, demands, actions, or causes of action that may arise as a result of my participation. This includes, but is not limited to, personal injury, property damage, or any other loss, whether caused by negligence or otherwise.
3. Indemnification: I agree to indemnify and hold Orlando Boom Inc. harmless from any claims, liabilities, damages, or expenses (including attorney's fees) incurred by them arising from my participation in basketball-related activities.
4. Medical Treatment: In the event of injury or illness during my participation, I authorize Orlando Boom Inc. to seek medical treatment on my behalf if I am unable to do so. I understand that I will be responsible for any medical expenses incurred as a result of such treatment.
5. Binding Agreement: This waiver is binding upon me, my heirs, executors, administrators, and assigns.
I have read this waiver and understand its content. I acknowledge that I am signing it freely and voluntarily, and I agree to be bound by its terms.
